    Brother/Sister

    Applicable only when there is no relative (spouse, lineal ascendant, direct descendant of spouse) of the patient, additional supporting documents must be submitted

    ※ When applying for a legal representative or voluntary representative of a relative, etc., the authorization between the patient's relative and the representative and additional submission of the representative's ID

  • Notes on issuance of copies of medical records

    Documents proving the kinship relationship with the patient: family relationship certificate, resident registration certificate, etc.

    • - Health insurance cards are not accepted because kinship is not proven.
    • - For accurate verification, please include the last digit of your resident registration number.
    • - Documents must be issued within 3 months.
    • - If the patient's consent cannot be obtained, if all relatives of the patient are absent, the right to apply is granted to the brother or sister.
    • - In the event that the only relative is unconscious and has no medical ability, the person selected as the guardian according to the Civil Act has the authority.
    • - If the patient agrees to appoint a sub-representative, the patient's relatives or representatives can be re-delegated (sub-representative).
    • - In the authorization written by the patient, it is necessary to specify the wording that the appointment of a sub-agent is allowed and a handwritten signature.
    • - Applicants under the age of 14 (if they have medical ability) must submit photo identification (passport, juvenile ID) in addition to the documents specified in Article 13-3 of the Enforcement Regulations of the Medical Act when applying for issuance of a copy as a relative or proxy.
    • - When writing the consent form, the scope of issuance (date, scope of recording paper, etc.) must be specified in detail.

    • Outpatient records (first visit, revisit), emergency records, discharge summary, hospitalization records, surgery records, doctor's orders, nursing records, initial nursing evaluation, examination result sheets (CT, MRI, ultrasound, tissue/cell, blood, heart), etc.
